Rates

Private Pay clients and out-of-network clients shall be in compliance with the No Surprise Act prior to services being provided. Compliance requires completion of consent and waiver forms and submission of a Good Faith Estimate (GFE) to the client. Please note this shall be done 72 hours prior to being seen by the provider.  Cancellations must be received within 24 hours of scheduled appointment. The cancellation notification must be received during normal business hours, (Monday-Thursday before 5:00 pm). If notification of cancellation is not received within the stated parameters, Strive will charge the card on file and assess a late cancellation / no-show fee. Late cancellation and/or no-show fees are assessed based on the cost of your session and strictly enforced. 

Payment

A credit card is required to reserve your session. 
Payment is due at the beginning of each session. We accept checks, cash, Visa, MasterCard, American Express and Discover and HSA.
